IL36RN

IL36RN，即白介素-36受體拮抗劑（Interleukin-36 receptor antagonist），也被稱為IL-1家族成員5（IL-1F5）。作為天然免疫調(diào)節(jié)因子，它通過阻斷IL-36與細胞表面受體的結(jié)合，維持皮膚、肺等組織的免疫穩(wěn)態(tài)。當該蛋白功能缺失時，IL-36介導的炎癥反應會異常增強，導致中性粒細胞過度浸潤和組織損傷。
IL36RN的遺傳突變與泛發(fā)性膿皰型銀屑?。℅PP）存在強關(guān)聯(lián)，超80%的家族性GPP患者攜帶其突變基因。臨床表現(xiàn)為突發(fā)高熱伴隨全身無菌性膿皰，嚴重時可危及生命。近年來針對這一靶點的藥物研發(fā)進展迅速，全球首個IL-36抑制劑斯佩格單抗（spesolimab）已于2022年獲FDA批準用于GPP急性發(fā)作，III期試驗顯示超半數(shù)患者在用藥一周內(nèi)膿皰完全消退。目前還有多個同類生物制劑和小分子藥物處于臨床試驗階段，為炎癥性皮膚病治療開辟了新方向。
