卵巢癌
卵巢癌是一種起源于女性卵巢的惡性腫瘤,根據(jù)組織來源不同,一般分為上皮性癌、生殖細(xì)胞惡性腫瘤、性索間質(zhì)細(xì)胞腫瘤,其中上皮性癌是卵巢惡性腫瘤中最常見的類型,約占90%。由于卵巢位于盆腔深部,腫瘤早期無特異性的癥狀,患者在就診時(shí),多已為晚期,預(yù)后較差,嚴(yán)重威脅患者生命安全。
卵巢癌的治療主要包括手術(shù)和化療,手術(shù)目的是切除腫瘤、明確診斷、準(zhǔn)確分期、判斷預(yù)后和指導(dǎo)治療。近年來,隨著對(duì)卵巢癌基因分子層面認(rèn)識(shí)的加深和靶向藥物逐漸應(yīng)用到臨床,卵巢癌的精準(zhǔn)治療手段也越來越豐富。
PARP抑制劑是近年來在卵巢癌治療中取得顯著進(jìn)展的一類藥物,它們通過抑制PARP酶活性,對(duì)BRCA突變或同源重組修復(fù)缺陷(HRD)的卵巢癌細(xì)胞具有殺傷作用。目前,多種PARP抑制劑如奧拉帕利、尼拉帕利、帕米帕利等已獲批用于卵巢癌的治療,尤其在BRCA突變或HRD陽(yáng)性的患者中顯示出較好的療效。此外,針對(duì)FRα的ADC藥物索米妥昔單抗(mirvetuximab soravtansine,MIRV)在卵巢癌治療中顯示出一定的效果。
