
世界胰腺癌日是每年的11月第3個(gè)星期四,旨在提高公眾對(duì)胰腺癌的認(rèn)識(shí)和關(guān)注。胰腺癌是一種發(fā)生在胰腺細(xì)胞中的惡性腫瘤,胰腺位于胃后方,負(fù)責(zé)分泌消化酶和調(diào)節(jié)血糖水平的激素。胰腺癌的病因尚未完全明確,但已知多個(gè)風(fēng)險(xiǎn)因素,包括吸煙、肥胖、糖尿病、慢性胰腺炎以及家族病史等。胰腺癌通常預(yù)后較差,5年存活率不足10%,早期癥狀不典型,往往在發(fā)現(xiàn)時(shí)已處于中晚期,因此被稱為“癌中之王”。胰腺癌的發(fā)病率在全球范圍內(nèi)居惡性腫瘤的第13位,死亡率居腫瘤相關(guān)死亡的第8位。
胰腺癌主要分為兩種類型:外分泌性胰腺癌(約占所有病例的95%)和內(nèi)分泌或胰島細(xì)胞瘤(也稱為胰島素細(xì)胞瘤),后者較為罕見(jiàn)。胰腺癌的早期癥狀不典型,可能包括上腹部不適或隱痛、消瘦、乏力等,容易被誤認(rèn)為是其他消化系統(tǒng)疾病。因此,加強(qiáng)胰腺癌早期診斷和治療,對(duì)于改善胰腺癌患者的預(yù)后具有重要意義。
胰腺癌的治療手段主要包括手術(shù)切除、化療、放療、免疫和靶向治療等。免疫治療是近年來(lái)胰腺癌治療的新焦點(diǎn)。目前,已有多個(gè)靶向不同靶點(diǎn),如IGF-lR、EGFR、HER2、VEGF、CTLA-4、PD-1的抗體藥物處于臨床研究階段 [1]。
